By selecting I Agree and signing below, I confirm that I am the participant or the parent/legal guardian of the participant, and that this consent applies to all AmazeX activities, including birthday celebrations, walk-in play, immersive experiences, and high-energy zones.

Acknowledgment & Assumption of Risk

I understand that AmazeX experiences involve active, immersive, and physical play, which carries inherent risks, including slips, falls, collisions, physical contact, and injuries that may be minor or serious.

I voluntarily choose to participate and assume all risks, whether known or unknown, foreseeable or unforeseeable.

Health & Conduct

I confirm that the participant is medically and physically fit to participate and will follow all safety rules, signage, and staff instructions.

I understand that unsafe or disruptive behavior may result in immediate removal without refund.

Parent / Guardian Consent (Minors)

If the participant is under 18, I confirm that I am the parent or legal guardian, consent on the minor’s behalf, and accept full responsibility for the minor’s participation and conduct.

Staff supervision does not replace parental responsibility.

Release & Indemnity

To the maximum extent permitted by law, I release and waive AmazeX from all claims arising from participation, including those due to ordinary negligence (excluding gross negligence or willful misconduct).

I agree to indemnify AmazeX for claims or damages arising from my or the minor’s actions.

Emergency Care & Media

I authorize AmazeX to provide basic first aid or arrange emergency medical assistance if required, and I accept full financial responsibility for related costs.

I consent to photography or videography for brand and promotional use unless declined in writing prior to participation.
